I would like to inform that  due to my health problem and family circumstances I am not in a position to continue to perform my duties and decided to take premature retirement ( under applicable  pension rules of the office). You are requested to relieve my after 90 days of notice period as per policy  of the (Company Name / Organization Name).
As per my records  no loans & advances are  recoverable from me, of the company (Office Name). Furthermore, I hereby authorize to recover, if any amount becomes due against me from the amount of  provident fund  / gratuity / leave encashment / commission / incentives / bonus / salary for the month payable to me. 
My twenty two years of  working with the (Company Name / Organization Name)  have been pleasant. I sincerely thanks the Management, my seniors and colleagues for the  support and cooperative given to me during this period of service.
You are requested to pay all the dues payable to me by crediting the amount in my Saving Bank a/c no. .... maintained at ABC branch of XYZ Bank and sanction disbursement of monthly pension as per rules of the (Company Name / Organization Name)
